3. Submit a full educational evaluation, which must have been completed within the past three years.
The following must be included in the evaluation.
- Educational Test Data: Individually administered tests of achievement
- Diagnosis of a Disability using DSM V Classification
- Psychological Test Data: Individually administered tests of ability (i.e. WISC)
4. Complete the appropriate standardized testing.
Middle School Applicants
Students applying for Middle School (grades 6, 7 or 8) are required to take the Independent School Entrance Exam (ISEE). Maryvale will host the ISEE testing on November 18, 2023. Specific instructions regarding registration, testing dates and sites can be found here.
Upper School Applicants
The HSPT will be offered on December 3 and December 10, 2022. Maryvale will administer the HSPT on Saturday, December 9, 2023.
Applicants should arrive at 8:30 a.m. for registration. Testing begins promptly at 9 a.m. and concludes at noon.

5. Send Maryvale your transcripts.
Freshmen Applicants Currently Attending a Catholic School
As part of the HSPT registration process, your school will receive a notification to send your transcripts and recommendations to Maryvale. No additional steps are needed on your family's end.
All Other Applicants
Download the Request for Transcript form and provide it to your current school after the close of the first grading period. This form gives the school your permission to send records from the past three years (grades, report cards, teacher comments and any/all standardized test scores) to Maryvale.

6. Send Maryvale your referral forms.
Freshmen Applicants Currently Attending a Catholic School
As part of the HSPT registration process, your school will receive a notification to send your referrals to Maryvale. No additional steps are needed on your family's end.
All Other Applicants
Two Common Referral forms should be completed by your daughter’s current teachers after the close of the first grading period. One form should be completed by her English/Language Arts teacher and the other by her Math teacher.
Maryvale is a Green School, so we kindly ask that you send the Common Referral forms directly to your daughter's teachers via the Maryvale Admissions Parent Portal after the application has been submitted. 8. Confirm the need of an interview.
After the Maryvale Admissions Committee reviews the above information, an interview may be required. If needed, Maryvale will contact the applicant's family to schedule an interview time.
